- Approve a Software Maintenance and Support Agreement with The Baldwin Group, Inc. for the Health Department Information System (HDIS) for $2,554.00 for the period of January 1, 2026 to December 31, 2026 (paid from fund 2313).
- Approve the FY26 Dental Sealant Program Agreement with Anna Mayle, Dental Hygienist, at an amount not to exceed $12,500.00 for the period of January 1, 2026 to December 31, 2026 (Clinic Dental fund 2320.303004).
- Approve a FY26 agreement with LaToya Dickens-Jones to provide Nurse Practitioner services on an as needed basis for the health department for an amount not to exceed $2,325.60 for a period of January 1, 2026 to December 31, 2026 (Nursing General Fund 7601.303001).
- Approve entering into a no-cost agreement with the Ohio Department of Health Outbreak Response Immunization Initiative to facilitate the ordering and distribution of ODH-issued pandemic or outbreak vaccine in the event a pandemic outbreak is declared from November 18, 2025 through September 30, 2030.
