2. Approve Amendment No. 4 to Contract No. 16538 with Lifeworks Services, Inc., for Day Support Services (DSS) and Employment Support Services (ESS) to extend the term of the contract through December 31, 2028, and add $57,000 per year for 2026, 2027 and 2028, for a new contract total of $287,000.
3. Approve Amendment No. 5 to Contract No. 16094 with Rise for Day Support Services (DSS) and Employment Support Services (ESS) to extend the term of the contract through December 31, 2028, and add $167,000 per year for 2026, 2027 and 2028, for a new contract total of $1,318,000.
