CONSIDERATION OF A GRANT APPLICATION IN THE AMOUNT OF $62,778 TO KDHE FOR SUPPLEMENTAL FUNDING FOR IMPLEMENTATION OF a chronic disease self- management program THROUGH THE HEALTH PROMOTION PROGRAM AT THE SEDGWICK COUNTY HEALTH DEPARTMENT. Presented by: Adrienne Byrne-Lutz, Interim Health Director, Sedgwick County Health Department RECOMMENDED ACTION: Approve the electronic submission of the grant application; and, if awarded, authorize acceptance of the grant and the establishment of budget authority as provided in the financial considerations section of this Request. Per Resolution 64-2013, this will be submitted to the County Manager for final approval and signature.
