CONSIDERATION OF A GRANT APPLICATION IN THE AMOUNT OF $12,965.00 TO PILOT AND IMPLEMENT A CHRONIC DISEASE SELF-MANAGEMENT PROGRAM THROUGH THE HEALTH PROMOTION PROGRAM AT THE SEDGWICK COUNTY HEALTH DEPARTMENT. Presented by: Claudia Blackburn, Health Director, Sedgwick County Health Department. RECOMMENDED ACTION: Approve the electronic submission of the grant application; and, if awarded, authorize acceptance of the grant and the establishment of budget authority as provided in the financial considerations section of this request. Per Resolution 64-2013, this will be submitted to the County Manager for final approval and signature.
