Document Title: "In-Reach" Nursing Home Services Contract <br>Submitted by: Heather O'Hayre <br>Department: Human Services <br>Description: The Department of Justice (DOJ) investigated Health, Care, Policy and Financing (HCPF) within the last 2 years and found that residents are inappropriately stuck in nursing homes due to a lack of resources or knowledge on how to transition back to the community. As a result, HCPF must now provide transition information through case management as part of the AAA structure for all nursing homes statewide. We have been funded to do this now for almost two years. A staff member from our Aging and Disability Resource Center team meets once with the resident to provide information on available resources. Weld and Boulder AAAs declined this service, so HCPF approached us last year to see if we would be interested in expanding our region. As a result, we are receiving an additional $161k to expand our service area.
