1. Approve amendment number 4 to the agreement with Lauderdale Motors, increasing the max cost by $20,000 per fiscal year, to provide repairs to the Sheriff's office vehicles and equipment on an as-needed basis.
2. Approve amendment number 1 to agreement with Willow Glen Care Center to increase the maximum cost and amend the exhibits and include rates for FY 26/27, for staffing and operation of Alpine House, a County-owned Adult Residential Facility.
3. Approve amendment number 1 to the agreement with Monique Dickinson, LVN, increasing the maximum cost to $40,000 per fiscal year, changing the term to auto renew and adding Attachment 1 to assist with managing the Jail Health Program under the direction of the Doctor and Physician Assistants.
4. Approve amendment number 1 to the agreement with Karen Corona, LVN, increasing the maximum cost to $92,000 per fiscal year, changing the term to auto renew, and adding Attachment 1, to assist with managing the Jail Health Program, under the direction of the Doctor and Physician Assistants.
5. Approve amendment number 2 to the agreement with Matthew Butler, P.A., increasing the maximum cost to $90,000 per fiscal year for 25/26 & 26/27 and $100,000 per fiscal year starting in FY 27/28, updating Exhibit B, and adding Attachment 1, to retain the services of a medical provider at the Trinity County Detention Facility and Trinity County Juvenile Detention Facility.
6. Approve amendment number 3 to the agreement with Donald Krouse, M.D., increasing the maximum cost to $27,000 per fiscal year, updating exhibits A and B, and adding Attachment 1, to provide supervision of medical professionals providing Jail Health services at the Trinity County Jail Facility and Trinity County Juvenile Detention Facility.
7. Approve amendment number 1 to the agreement with Wineglass J Vineyards/Jon Paladini, modifying certain provisions of the Lease, including the Commencement Date, responsibility for design and construction of interior tenant improvements, reimbursement procedures, and certain construction cost adjustments to provide the lease of real property located at 35 Arbuckle Court in Weaverville.
