a. Approve and authorize the Director of Health Services or designee to execute Amendment No. 2 to the Agreement with Greg Facktor & Associates, LLC for the provision of Federally Qualified Health Center (FQHC) finance and compliance consulting services, increasing the contract amount by $500,000 for a revised contract amount not to exceed $1,100,000 with no change to the contract term of January 1, 2026 through June 30, 2028; and b. Approve and authorize the Director of Health Services or designee to execute up to three future amendments that do not in total exceed 10% ($40,000) of the original agreement, do not significantly change the scope of services, and do not increase the total not to exceed amount over $1,140,000; and c. Approve and authorize the County Purchasing Agent or designee to execute future Amendments to the agreement where the Amendments do not significantly change the scope of work and fall within the signing authority of the County Purchasing Agent.
