a. Approve and authorize the Director of Health Services or designee to execute Amendment No. 5 to the Paramedic Service Provider Agreement No. A-12329 between the County of Monterey and the City of Carmel-by-the Sea, to extend the term of the agreement for one and a half years, for an aggregate term of October 5, 2012 through June 30, 2028, for the provision of paramedic services in the County of Monterey; and b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments that do not significantly change the scope of services.
