a. Approve and authorize the Director of Health Services or designee to sign a Standard Agreement (Agreement) with Montellese Chiropractic PC (Montellese) to arrange for and furnish the services of duly qualified and licensed chiropractor(s), with a term of two years of June 23, 2026 to June 30, 2028 and a contract amount not to exceed $880,000; and b. Authorize the Director of Health Services or designee to sign up to three future amendments to this Agreement where each amendment does not exceed 10% ($88,000) of the total contract amount and does not significantly change the scope of work.
