a. Approve and authorize the Director of Health Services or designee to execute an Agreement with the County of Santa Cruz, for the provision of administrative and fiscal intermediary services by Santa Cruz County as Host Entity for the County-Based Medi-Cal Administrative Activities (“CMAA”) and/or Targeted Case Management (“TCM”) programs, for the period retroactive from July 1, 2025 through June 30, 2028, for the payment to Host Entity of a one-time fee of $500 for membership in the CMAA and TCM Local Government Agency (“LGA”) Consortium, and annual compensation to the Host Entity in an amount to be determined based on the approved Consortium annual budget; and b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future Amendments to this Agreement that do not significantly change the scope of work.
