a. Approve and authorize the Director of Health Services or designee to execute Amendment No. 2 with Clear Labs, Inc., for the purchases of a service plan for a Clear Dx System, Clear DX WGS SARS--CoV-2 Reagents, and Clear Dx Microbial Surveillance Reagents and extend the term for an additional two years, for a new term of May 17, 2022 through March 31, 2027, and a total Agreement amount not to exceed $1,017,641; and b. Approve and authorize the Director of Health Services or designee to sign up to three future amendments where the combined amendments do not exceed 10% ($101,764) of the original amount and do not significantly change the scope of services, total not to exceed a maximum of $1,119,405.
