a. Approve and authorize the Director of Health Services or designee to execute an agreement with CE Broker, Inc. dba Propelus for automated license verification services for all Health Department Bureaus, for an amount not to exceed $15,660 with an agreement term retroactive to October 8, 2024 through October 7, 2027; and b. Approve the Director of Health Services’ recommendation to accept all non-standard provisions in agreement, including indemnification; and c. Authorize the Director of Health Services or designee to execute up to three (3) future amendments and/or Order Forms to this agreement where the total amendments do not exceed 10% ($1,566) of the original contract amount, do not significantly change the scope of work, and do not exceed the maximum amount of $17,226; and d. Approve and authorize the Contracts and Purchasing Officer or designee to execute future amendments and/or Order Forms, provided they do not exceed an aggregate amount of $200,000 and fall within the signing authority of the Contracts and Purchasing Officer.
