a. Authorize the Director of Health Services or designee to execute Amendment No. 1 to Program Funding Agreement No. 7480-CA CCE-CP_2064_MONTEREY-01-G issued by Horne LLP with no change in the agreement term or dollar amount due to the California Department of Social Services (DHCS) reassigning Community Care Expansion Preservation Projects (CCE) third-party administrative duties to Horne LLP; and b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future amendments where the combined amounts do not exceed 10% ($320,074) of the original Agreement amount, do not significantly alter the scope of services, and do not increase the total not to exceed amount over $3,520,818.
