a. Approve and authorize the Chair of the Board of Supervisors, or designee, to sign Amendment No. 1 to Agreement A-16373 with the Central California Alliance for Health to provide health plan benefits for eligible In-Home Supportive Services providers, adding $4,927,855 for a new contract total of $10,407,536, extending the term by one (1) year, for a new term of July 1, 2023 to June 30, 2025, including non-standard termination and mutual indemnification provisions; and b. Authorize the Chair of the Board of Supervisors, or designee to sign up to three future amendments to this amended Agreement where the total amendments do not exceed 10% ($1,040,754) of the amended contract amount, and do not significantly change the scope of work, and do not exceed the maximum aggregate amount of $11,448,290.
