Authorize the Director of Health Services or designee to sign Amendment No. 2 to Agreement with Clinicians Telemed Medical Group, Inc, (CT) for qualified and licensed telehealth physicians, nurse practitioners, physician assistants, and/or clinical social workers, extending the term one (1) additional year for a new term of September 1, 2022, to June 30, 2025, with no changes to the total contract amount of $98,000; and b.
