a. Approve and authorize the Director of Health Services or designee to execute an Agreement with the County of Santa Cruz, for the provision of administrative and fiscal intermediary services by Santa Cruz County as Host Entity for the County-Based Medi-Cal Administrative Activities (“CMAA”) and/or Targeted Case Management (“TCM”) programs, for the period retroactive from July 1, 2022 through June 30, 2025, for the payment to Host Entity of a one-time fee of $500 for membership in the CMAA and TCM Local Government Agency (“LGA”) Consortium and an annual compensation to Host Entity of $71,500, for a total Agreement amount of $215,000; and b. Approve and authorize the Director of Health Services or designee to execute up to three (3) future Amendments to this Agreement that in total do not exceed 10% ($21,500) of the original Agreement amount, do not significantly change the scope of work, and do not exceed a revised maximum amount of $236,500.
