a. Approve and authorize the Director of Health Services or designee to execute a Standard Agreement with Alliance on Aging to provide monthly caregiver training classes, caregiver support group sessions and chronic disease prevention classes and to integrate information on brain health and cognitive decline risk factors into existing health communications, effective upon execution through June 30, 2025, for a total Agreement amount not to exceed $340,000; and b. Approve and authorize the Director of Health Services or designee to sign up to three amendments where the combined amendments do not exceed 10% ($34,000) of the original contract amount and does not significantly change the scope, total not exceed maximum of $374,000.
