Approve a Grant Agreement with the Sierra-San Joaquin Perinatal Health Program to Provide Services as Aligned in the Grant Award; Authorize the Board of Supervisors Chair to Sign the Grant Agreement; and Authorize the Health and Human Services (HHSA) Director to Sign Any and All Future Amendments Pertaining to this Grant (Subject to Approval as to Legal Form by County Counsel)
