Approve and authorize the Chairman to execute a revenue agreement with the California Department of Public Health for the Type 2 Diabetes Prevention Program to address chronic disease prevention, effective upon approval of California Department of General Services through June 30, 2028, total not to exceed $237,000; and approve and authorize the Director of the Department of Public Health, or designee, to execute the Contractor’s Release for the final invoice and other forms as required by the proposed Agreement
