(IDEXX), for veterinary medical supplies with no change to the not-to-exceed amount of $60,000 or terms and conditions of the contract, and to extend the term an additional six months, for a new term of January 1, 2021, through June 30, 2025; 3) Authorize the Purchasing Agent to execute amendments relating to Agreement 5357, contingent upon approval by County Counsel and Risk Management, which do not increase the maximum dollar amount or term of the Agreement; and 4) Authorize the HHSA Director, or the Chief Assistant Director, to execute programmatic, administrative, and fiscal documents relating to Agreement 5357. FUNDING: 15% Fees for Services, 9% Licensing/Permit Fees, 7% Public Health Realignment, 69% Net County Cost, and in some cases the services are 100% paid for through the Pet Aid Special Revenue Fund.
