1. Approve and authorize the Director of Health and Human Services to sign the attached renewal Agreement with Laboratory Corporation of America Holdings to continue providing genetic testing services for the term of December 1, 2025 through November 30, 2027, as requested by the Director of Health and Human Services.**
2. Approve and authorize the Chair to sign the attached MOU renewal with Open Door Community Health Centers to continue Public Health’s Community Health Education Kiosk Project for the term of November 1, 2025 through June 30, 2027, as requested by the Director of Health and Human Services.**
3. Approve and authorize the Chair to sign the attached renewal Agreement with California Hearing Officers, LLP to continue providing Hearing Officer Services for the term of January 1, 2026 through December 31, 2026, as requested by the Director of Health and Human Services.**
4. Approve and adopt Budget Transfer #11-01 in the amount of $26,871.00 to the Building Maintenance & Parks budget for FY 25/26, adding $26,871.00 for the purchase of a replacement vehicle from the insurance reimbursement revenue to be received from Trindel and authorize the purchase of a 2026 Chevrolet Trailblazer at a price not to exceed $26,871.00 as requested by the Director of Building Maintenance & Parks**
5. The Chair requests any deletions, corrections or additions from Board members to the agenda at this time. In order to add an item to the agenda, the matter must have come to the attention of the County subsequent to the posting of the agenda and the matter requires action before the next regular meeting of the Board of Supervisors.
6. 10:30 A.M. Hold a public hearing to consider the Hasmik Tamazyan Rezone (R2501C) located on Old Mill Road, APN 116-060-011. It is recommended that the Board of Supervisors take action by: 1) Adopting the Mitigated Negative Declaration and the staff findings outlined in the staff report; 2) Approving the rezone as described in Exhibit “A” by approving an ordinance amending Coastal Zoning Map B-9; and 3) Adopting a resolution forwarding an application to the California Coastal Commission for a Local Coastal Program (LCP) minor amendment, and direct staff to prepare the application as requested by the Planner of the Community Development Department.**
7. Approve and authorize the Chair to sign the attached Lease Agreement for Behavioral Health clients with Crescent City Church of the Nazarene for the term of December 1, 2025 through December 31, 2026, as requested by the Director of Health and Human Services.**
