1. Approve and authorize the Chair to sign the attached MOU renewal with the Yurok Tribe to continue Public Health’s Community Health Education Kiosk Project for the term of November 1, 2025 through June 30, 2027, as requested by the Director of Health and Human Services.**
2. Approve and authorize the Director of Health and Human Services to sign the attached Performance Contract renewal with the California Department of Public Health for the term of July 1, 2026 through June 30, 2029, as requested by the Director of Health and Human Services.**
3. The Chair requests any deletions, corrections or additions from Board members to the agenda at this time. In order to add an item to the agenda, the matter must have come to the attention of the County subsequent to the posting of the agenda and the matter requires action before the next regular meeting of the Board of Supervisors.
4. Approve and authorize the Chair to sign the attached second Amendment to the Agreement for Medi-Cal Peer Support Specialist Certification Program with CalMHSA for an additional one-year term of May 27, 2026 through May 27, 2027, as requested by the Director of Health and Human Services.**
5. Approve and authorize the Director of Health and Human Services to sign the attached Conditional Award Attestation Letter for the upcoming Behavioral Health Continuum Infrastructure Program Round 2 Grant Agreement with the California Department of Health Care Services, as requested by the Director of Health and Human Services.**
6. Approve and authorize the Chair to sign the attached Agreement between Liberty Vote and Del Norte County continuing the lease agreement of our current voting equipment as requested by the Registrar of Voters.**
